Emergency Information
Emergency contact and medical information must be submitted on-line. We are no longer using paper forms.

We will NOT accept paper forms by mail or fax.
First-time users need to register. Information is saved and can be updated thereafter using the logon you create. Your e-mail address is required (it will be designated as your unique username).
If you do not have an e-mail address, use firstname.lastname@nomail (substituting your first and last name), however, you will not be able to receive e-mail confirmation of your registration and changes.
If anyone does not have access to the internet, we are asking state coordinators to help us by entering information for these attendees. If you entered your information last year, please update any changes now. If there are no changes, you must still re-submit your data, to let us know that you are attending and that your information is still valid.
