I used to use the hall pass "sheet" also, but then our principal said the students had to do something academic for extra credit or to earn a homework pass.

I use kind of a combination. I give them passes (high school) that last the quarter and they get 3, 2, or 1 point their final grade. They pay attention to that! Then if they use up their 3, I have them sign the clipboard with where and when. Then, when I want to say, "You go every other day" or "always at 12:20" I have proof. I'm not sure why, but they don't like writing their name and time. This has been my pet pet peeve for 25 years. I just can't understand how they can not be en rapt and have to drag themselves away :D.

I must admit that they almost never ran out. If they did, I gave them a way to get another one, such as I would ask: what is you fav class? or your fav teacher? So of course they said math or my name. It wasn't a do or die rule, but it controlled alot of their behavior. My main focus was teaching bell to bell and eliminate the interruptions but you have to have have everything all planned out in your classroom management, grading system. Of course, in an emergency, they were allowed to use the rest room.

What did you do when they ran out? Did you tell them they could not go? I have tried this and even with 10 or more passes a quarter one or two kids use them up right away then wiglle and squirm and act like they are gonna have an accident---6th graders! So, I gave up! Yes, I also had a good incentive- a can of pop and other items for x number of remaining passes.

I used Homework Passes. I made up a sheet with 4 on it and a place for students to write their name, date, time and destination. If they needed to use the restroom, go to locker, etc as long as new work was not being taught, they raised their hand and I signed off on the pass, no tearing up or cutting it off. They had one sheet per quarter to use up, or turn in for a few points if not used, wrinkled, clean etc. This cut down on students leaving the room ALOT as they wanted the points so unless it was an emegency, most students chose NOT to use up the passes. Worked like charm. Then the next quarter, they got a new sheet, different color made record keeping easy. I share this all the time with new teachers I mentor.
